A fortnight after collapse of the railway bridge in Mumbai's Andheri on July 3, Commissioner of Railway Safety in his report has said the accident was a failure on part of the staff.
One person had died due to the collapse of the bridge and four others were grievously injured.
Commissioner of Railway Safety Sushil Chandra cited pitting and corrosion of cantilever and steel brackets of the pathway as causes for the collapse. Additional load put on by BMC (Brihanmumbai Municipal Corporation) like cables, sand and paver blocks without permission from Western Railway also added to the collapse.
